Congratulations on winning a District ADDY® Award!
If you have won a District Gold ADDY, your winning entry has been forwarded to the national judging and the fee has been paid by your district. Your entry will be automatically included in the judging of the National ADDY Competition.
If you have won a District Silver ADDY and would like to enter it in the judging of the National ADDY Competition, please click on the link below and fill out the form so we know which entry/entries you would like to be judged. Once you have filled out the form, please mail it to the address listed on the form WITH a check to cover the forwarding fee ($100 per professional entry, $20 per student entry), or include credit card information and mail or fax it to the number listed on the form. You do not need to re-send the entry itself; that has already been sent to us by your district. You only have to pay the entry fee to have your work judged.
All forms and payments must be received by 5:00pm EST on Friday April 15, 2013 to be included in the National ADDY Competition.
